Job Description:
Responsible for managing an assigned caseload of ASAM Level of Care 2.1 (Intensive Outpatient) and Level of Care 3.5 (Clinically Managed Residential) patients diagnosed with a substance use disorder and/or comorbid mental health conditions. Assesses patient needs; develops, implements, and reviews service plans; and works with other community resources to meet/achieve patients' service needs.

Job Responsibilities:
• Accepts newly assigned cases for which diagnostic intake has been completed by a licensed provider and subsequent referral made for targeted case management.
• Completes case management assessments on all patients entering caseload. Observes, records, and reports on an individual's functioning, documenting all information within the identified electronic health record within established agency guidelines.
• Develops appropriate treatment/service plans with patients as the means for implementing appropriate services and identifying appropriate external referrals, as needed.
• Provides ongoing supportive and/or case management functions in accordance with the problems, needs, and strategies identified in the service plan to help patient achieve the stated goals and objectives.
• Provides face-to-face reviews with patients and/or relevant staff on a regular basis to report the progress made in reaching service goals.
• Modifies plans as necessary to ensure that goals and objective are achieved.
• Participates in planning and service coordination activities to improve and enhance service continuity and effectiveness for patients, to include but not be inclusive of referrals to interagency programs.
• Develops a comprehensive knowledge of substance use disorders and meeting the needs of patients to provide holistic care.

Required Qualifications:
• Bachelor's degree in a human services field (i.e., psychology, social work, sociology)
• Demonstrated ability to interview and assess patients using appropriate assessment tools.
• Ability to read and understand biopsychosocial assessments and use them in developing treatment plans.
• Familiarity with and ability to identify community resources for patients and to coordinate provisions of services within Sumter County.
• Ability to establish effective working relationships with internal agency staff as well as with relevant community organizations.
• Knowledge and passion for working with individuals with substance use or mental health disorders.
