Job Description: Provides supportive case management services to clients receiving counseling and behavioral health services. Assesses client needs, coordinates care, links clients to community resources, advocates for services, and supports clients in achieving their recovery or treatment goals. Works as part of a multidisciplinary team to ensure comprehensive, client-centered care.
Job Responsibilities:
• Conducts initial intake paperwork.
• Coordinates referrals to internal and external services, such as housing, employment, health care, substance use treatment, and social services.
• Monitors client progress, updates service plans regularly, and adjusts support as needed.
• Collaborates with therapists, medical providers, peer support specialists, and other professionals involved in the client's care.
• Advocates on behalf of clients to ensure access to needed services and benefits.
• Maintains accurate and timely documentation in accordance with agency and funder requirements.
• Participates in team meetings, trainings, and case consultations.
• Provides crisis intervention support or assists clients in accessing emergency services when appropriate.
Support client transitions (e.g., from inpatient to outpatient care, reentry from incarceration, or discharge planning).
Answering phones and scheduling appointments
Collecting specimens for drug screening
Required Qualifications:
• Strong organizational and communication skills
• Ability to build rapport and maintain professional boundaries
• Knowledge of community resources and support systems
• Comfortable working with diverse populations and individuals with complex needs
• Ability to work independently and as part of a team
• Valid driver’s license and reliable transportation (if fieldwork is required)
• Background check and clearances (per agency policy)
Preferred Qualifications:
• Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
• At least one year of experience in case management, social services, or behavioral health OR certification as a Peer Support Specialist
• Experience with electronic health records (EHRs)
